The Movistar Rider threw down the gauntlet, after breaking from the chief contenders on Giro's toughest hill so far, with Britain's Simon Yates 1 minute and 32 seconds behind, after attacking on the last kilometer.
Vinchenzo Nibali came in third, 1 minute and 54 seconds off the pace.
In the general classification, Carapaz now leads Primosh Roglich by seven seconds, with Nibali back in third, one minute and 47 seconds adrift.
