Differentiating culture from religion

Religion and culture are intertwined to the point that they seem indistinguishable.

Yet, we see communities that have migrated to significantly different cultures questioning, challenging and evolving their own practices to deal with different practical realities.

Does this mean that cultural practices get in the way of religion? How can Muslims differentiate between religion and culture?
